  4. Ask every new client to fill it in before the first service, and go through it again when her health or nail history changes.

Questions

What should a nail client consultation form include?

Contact details, health questions (allergies, skin conditions, diabetes or circulation problems, pregnancy, medication, infections), the condition of the natural nails and any enhancements, preferences such as shape and length, a service history table and a signed consent statement. This generator includes all of them.

Is this form a medical assessment?

No. It helps you decide whether a service is safe and when to suggest that a client sees a doctor. It does not diagnose anything, and the consent statement says so.

Do you store what I type?

No. The form is built in your browser. Your salon name, details and logo are not sent to our servers and are gone when you close the page.

How should I store signed forms?

Health details are sensitive personal data in many countries, for example under the GDPR in Europe. Ask only what you need for the service, keep signed forms in a locked drawer or a protected digital folder, and limit who can read them.

How often should clients update the form?

Before the first appointment, and again whenever something changes: a new medication, a pregnancy or a reaction after a service. Many salons also ask for a fresh signature once a year.

Can I keep consultation forms in CutieCure Salon?

CutieCure Salon has no built-in consultation or consent forms. Keep signed forms in your own paper or digital file, and use the client notes in the dashboard for the short working notes your team needs.
